Medway Diabetes

Medway Diabetes provides a single electronic solution for recording the health records of diabetes patients.

The system supports use by GPs and hospital doctors, clinic and specialist diabetes nurses, dieticians, and specialists from chiropody and optometry services so information is shared between professionals and changes in the patient's condition can be monitored.

Medway Diabetes uses an integrated care pathway to collect information from all associated carers.  It can be used both for one-off visits and for multi-disciplinary team working. Each user is provided with instant access to a patient's complete medical history, and sees current and previous results in the context of the patient's visit.
